"Patients who benefit the most from home visits are usually bed-bound and uncommunicative.  They tend to be more receptive when the care team provides follow up treatment in the comfort of their homes. Caregivers, too, feel more reassured by the visits knowing that they are not alone in this journey. 

To enable our patients to be cared for adequately at home and in the community, we often need to look beyond our hospital walls and pool resources with community partners such as ILTC agencies and VWOs.

Having good relationships with community partners also helps us to better understand and address issues concerning care in the community."
