Singapore, 6 April 2018 – As part of ongoing efforts to strengthen community care, SingHealth has set up eight Community Nurse Posts, located at Senior Activity Centres, where more than 30 nurses from Singapore General Hospital (SGH) and Changi General Hospital (CGH) provide healthcare services to vulnerable elderly in the community to enable them to keep well and age well.
Set up in collaboration with community partners, SingHealth Community Nurse Posts are strategically located at Senior Activity Centres (SACs) in the southeast and east of Singapore to enable elderly with multiple chronic diseases to receive care closer to their homes. The Posts are led by Registered Nurses trained in specialties such as chronic disease management, geriatrics, oncology or palliative care.
As of 31 March 2018, more than 260 elderly residents have visited SingHealth Community Nurse Posts for their health needs.
Types of Services
- Health and geriatric assessment
- Health coaching for disease prevention
- Chronic disease monitoring
- Education on self-care, medication and chronic disease management
- Medication reconciliation and consolidation
- Provision of early interventions to prevent condition from worsening
- Home visits (where necessary)
- Care coordination with Senior Activity Centre and other care agencies to provide social care support
