​"Most patients want to go home as soon as they can.  Being able to recuperate in familiar surroundings and being close to loved ones gives them great comfort.  They're also happier and more motivated to get well!  Helping to make this happen involves a lot of work and coordination between various health and social care providers, especially for patients with complex care needs.  My greatest satisfaction is being able to prepare them to return home and making sure that post-discharge care fully meets their needs."
