"Some patients tend to put off seeking help until their condition becomes really severe, when they have no choice but to go to the A&E. No one should be in such a state – sickly, helpless, or even dying alone.   Studies we conducted show that a home care team can reduce A&E visits, hospital readmissions and length of stay by 30 per cent on average.   As clinicians, we need to step out of the hospital and work closely with community partners to keep at-risk persons healthy and able to lead dignified lives at home."
