​"As a liver pharmacist, I am involved in the patient care process when he or she is accepted for pre-transplant work up where they are admitted to the hospital for a week. During this period, we ensure that patient’s medications are optimised before their transplantation.

We plan the medication routine with our patients during the counselling sessions and educate them on why they need the medication.  There are some who require more time to adjust to the routine, or even experience organ rejection when they couldn’t adhere to their medication schedules.

Some patients or caregivers will come up with creative ways to remind themselves such as setting up family chat groups or creating attractive charts."
