DOT right-sites patients with stable chronic conditions from Specialist Outpatient Clinics at Singapore General Hospital and National Heart Centre Singapore to 150 GPs islandwide.

As part of the Network, GPs can look forward to shared resources such as:
- Care coordinators to help GPs monitor their patients' health, arrange for regular screenings, coordinate with allied health providers, and follow up when they receive treatments at SingHealth institutions.
- A chronic disease registry of their patients, to track and monitor care outcomes easily
- Ancillary services like diabetic eye and foot screening for patients with more complex conditions
- Direct access to select specialist services provided at SingHealth institutions
Dr Lily Aw, GP Lead of the DOT PCN said, "I'm confident that a strong partnership between GPs and SingHealth, through the Network, will help us to deliver more seamless care to patients."
The signing ceremony, led by the PCN EXCO members, was witnessed by Prof Ivy Ng, Group CEO, SingHealth, Dr Lily Aw, GP Lead of the DOT PCN, and Dr Edwin Low, Group Director, SingHealth Regional Health System.
